Your First Visit
If you have any questions or would like to book an appointment please call us at 604-510-9900 or email firstname.lastname@example.org.
A doctor’s referral is not required for private physiotherapy services; however, some private health insurance companies do require a referral. Payment is due after each treatment session and you will be provided with a receipt to submit to your private insurer for reimbursement. Please enquire with your health insurer if you have any questions regarding re-imbursement.
Please note we are a private clinic and we do not accept WorkSafe BC funded clients. If you are a WSBC patient living in Langley and wanting physiotherapy treatment, please call and we can refer you to an appropriate physiotherapy clinic.
What to Expect
At Pure Form Physio, physiotherapy appointments are usually 30 minutes long and always one-to-one; your therapist will not be working with other patients during your appointment. The initial appointment is focused on understanding your goals, concerns, performing a thorough assessment and beginning treatment. Treatment options include manual therapy, Intra-muscular stimulation (IMS), massage / myofascial release, running gait re-training and exercise rehabilitation.
What to Wear
If you are having a treatment of the low back or lower limbs please bring a pair of shorts. For female physiotherapy patients that are having a shoulder, upper back or neck treatment please bring a tank top/singlet. We have shorts and tank tops for people that need to borrow these items for their appointment. You will have time to change into them at the start of the appointment.
What to Bring
- Any referral notes or letters from any other healthcare professional
- Any medical imaging results (e.g. MRI, x-ray, CT scan , ultrasound results)
- A list of all medications
- Runners should bring the shoes they currently run in
If you have been recently injured in a motor vehicle accident you will have coverage for physiotherapy treatments through ICBC. You can begin physiotherapy treatment without a doctor’s referral, but ICBC may request one at a later date. We will assist you in confirming your coverage and will contact ICBC when necessary. It is important for you to maintain communication with your adjuster to ensure coverage is in effect.
To avoid a late cancellation or missed appointment fee, we kindly ask that if you need to cancel or change your appointment please contact reception with at least 8 business hours notice.
We will not be charging late cancellation fees at this time if you are feeling unwell.
Parking and Access
There is ample free parking, including a reserved parking spot for people with mobility limitations in front of the clinic. The entire clinic is wheel chair accessible.
